Who Are The Professionals Sent To The Care Recipient's Home

  • Registered Nurses (RNs) providing individual care and case management for medically prescribed treatment.
  • Licensed Vocational Nurses (LVNs) working under a physician's order and RN supervision will administer medication, treatment, and assist the patient with returning to independent activities of daily living.
  • Certified Home Health Aides provides personal care
    and/or light house-keeping.
  • Physical Therapists, Occupational Therapists and Speech Therapists provide rehabilitative services in order to assist patient's return to higher levels of functioning.
  • Medical Social Services access community resources to acquire the assistance needed by patients.
  • Dieticians assist the patient's to follow and plan for the proper diet ordered by the physician  

Once a nurse or a clinical representative of Excellent In-Home Care, Inc. receives the referral information, the patient will be evaluated within 48 hours or on the date specifically ordered, a professional clinician or team of clinicians will determine the proper plan of care and will begin to administer that care.

The Skilled Nurse provides skilled care and instructions to patients and their families. They enable the patient and family to function independently at home, increasing the comfort of the patient and improving overall health.
